Terms and Conditions for the Use of Medical Vouchers in OpenMed Medical Centre Facilities

1. General provisions

1. These Terms and Conditions set out the rules for issuing, using and redeeming medical vouchers (hereinafter: the "Vouchers") offered by OpenMed Centrum Medyczne Sp. z o.o., with its registered office in Warsaw, at ul. Wiatraczna 25, unit u2, 04-384 Warsaw, entered in the Register of Entrepreneurs of the National Court Register kept by the District Court for the Capital City of Warsaw, 13th Commercial Division of the National Court Register, under KRS number: 0000718744, NIP: 1132963150, with share capital of PLN 10,000.00, operating facilities under the brand OpenMed Medical Centre.

2. Vouchers may be redeemed exclusively in OpenMed Medical Centre facilities operated by OpenMed Centrum Medyczne Sp. z o.o.

3. These Terms and Conditions have been drawn up on the basis of the applicable laws of the Republic of Poland, including consumer protection regulations and regulations governing the provision of healthcare services.

4. These Terms and Conditions are made available to the Client prior to the purchase of a Voucher, in particular on the website www.openmed.pl and in facilities operating under the OpenMed Medical Centre brand.

2. Types of Vouchers

1. Vouchers are available in two variants:

a) percentage-based – entitling the holder to use a medical service with a discount expressed as a percentage,
b) value-based – entitling the holder to a medical service with a value equal to the value of the Voucher or higher; if the price of the service exceeds the value of the Voucher, the difference must be paid by the Client.

2. The type of Voucher, its validity period and any additional conditions of use are specified on the Voucher itself or in the information provided to the Client at the time of issuance.

3. Issuance and validity of Vouchers

1. Vouchers are issued by OpenMed Centrum Medyczne Sp. z o.o. as:

a) a product purchased by the Client, or
b) a voucher awarded to the Client by the Company, including within promotional programmes, special offers or commercial decisions.

2. Value-based Vouchers may only be redeemed after full prepayment by the Client.

3. A Voucher is valid for 12 months from the date of its issuance. After this period, an unused Voucher expires and cannot be redeemed or refunded.

4. Failure to use the Voucher within its validity period results in its expiry; any unused amount is non-refundable and cannot be exchanged for cash.

4. Redemption of Vouchers

1. Vouchers may be redeemed exclusively in OpenMed Medical Centre facilities operated by OpenMed Centrum Medyczne Sp. z o.o.

2. Redeeming a Voucher requires:

a) informing the reception team of the intention to use a Voucher at the time of booking the appointment,
b) presenting the Voucher (in paper form) before making payment for the service.

3. Vouchers cannot be exchanged for cash and do not constitute a means of payment.

4. A Voucher may be used once, unless its terms specify otherwise.

5. Vouchers cannot be combined with other promotions or discounts unless the terms of a specific promotion state otherwise.

6. Voucher redemption is subject to the availability of appointment slots with doctors and medical staff. A Voucher does not guarantee an appointment at a specific date and time; it entitles the holder only to receive a medical service subject to the facility's availability.

5. Complaints and liability

1. If a Voucher is lost or damaged, a duplicate may be issued provided that OpenMed Centrum Medyczne Sp. z o.o. can clearly confirm that the Voucher has not been redeemed.

2. The Company will review any complaint within 30 days of receipt.

3. Matters not regulated by these Terms and Conditions are governed by the applicable provisions of Polish law, in particular the Civil Code.

4. In the case of a Voucher purchased remotely, the Client has the right to withdraw from the contract within 14 days from the date of purchase, provided that the Voucher has not been redeemed.

6. Amendments to the Terms and Conditions

1. OpenMed Centrum Medyczne Sp. z o.o. reserves the right to amend these Terms and Conditions.

2. The Company will inform Clients of any amendments by publishing the updated version on the website www.openmed.pl.

3. Amendments to the Terms and Conditions do not affect the acquired rights of Clients holding valid Vouchers. Vouchers purchased prior to the amendments are governed by the Terms and Conditions in force on the date of purchase.

7. Effective Date

These Terms and Conditions shall enter into force on 9 December 2025.
