Acne diagnosis and treatment
Acne is one of the most common skin diseases, affecting not only teenagers but also increasingly adults. Contrary to appearances, it is not just an aesthetic problem. Untreated acne can lead to scars and permanent discoloration, as well as negatively affect self-esteem.

What does acne diagnosis look like at OpenMed?
Effective treatment begins with proper diagnosis. During the visit, the dermatologist assesses:
- type and severity of lesions (comedones, papules, pustules, cysts, erythematous changes)
- possible causes: hormonal disorders, stress, diet, inappropriate care, environmental factors, and medications
- presence of post-acne scars or post-inflammatory hyperpigmentation
- skin type and tolerance to treatment
If necessary, tests such as hormones (androgens, prolactin, TSH), glucose, insulin, complete blood count, vitamin D, as well as endocrinological, gynecological, or dietary consultation may be ordered if the clinical picture indicates this. This approach allows treating the cause, not just visible symptoms.
When is it worth seeing a dermatologist?
It is worth consulting a dermatologist at any stage of acne problems, especially when acne is very severe. Even the first inflammatory changes or comedones are a signal that the skin needs help. Adult acne particularly requires urgent consultation, as it often has a hormonal basis and can persist for years. It is also worth remembering that the earlier we start treatment, the greater the chance of avoiding scars and discoloration.

Types of acne
Proper diagnosis is the key to effective treatment. Below is a full description of the four main types of acne we most commonly work with at OpenMed:
1. Acne vulgaris
The most common form of acne, characteristic of adolescence but increasingly observed in adults as well. Lesions include:
- open and closed comedones
- papules and pustules
- cystic lesions (in more severe forms)
- tendency to scarring
Treatment: topical retinoids, azelaic acid, benzoyl peroxide, topical antibiotics; in more severe forms, oral antibiotics or isotretinoin.
2. Rosacea
A chronic skin disease in adults, completely different from acne vulgaris. Characteristic symptoms:
- facial erythema
- telangiectasias (broken blood vessels)
- papules and pustules
- skin hyperreactivity
Aggravating factors: alcohol, spicy foods, stress, sun, inappropriate care.
Treatment: preparations reducing erythema, topical antibiotics, metronidazole, ivermectin, oral therapy, and vascular treatment (e.g., laser).
3. Hormonal acne
Most commonly affects adult women, especially those with:
- irregular cycles
- PCOS (polycystic ovary syndrome)
- insulin resistance
- androgen disorders
- stress and sleep disorders
Symptoms:
- lesions mainly in the lower part of the face (jawline)
- deeper, painful papules and nodules
- perimenstrual exacerbations
Treatment: dermatological pharmacotherapy, hormonal treatment (e.g., anti-androgen), low GI diet, regulation of circadian rhythm and stress.
4. Cosmetic acne
Appears as a result of using too heavy or comedogenic cosmetics, too many products at once, or improper care. Typical symptoms:
- numerous small comedones
- subcutaneous papules
- worsening after changing cosmetics or care
Treatment: elimination of comedogenic products, introduction of "skinimalism", appropriately selected care, and possible anti-inflammatory treatment.
Acne treatment methods: topical, systemic, and combined therapies
Acne treatment is always individual and may include several methods simultaneously. Depending on the type and severity of lesions, we use:
- Topical medications: creams and gels containing retinoids, antibiotics, benzoyl peroxide, or azelaic acid
- Systemic treatment: in more severe cases, the dermatologist may include oral antibiotics, isotretinoin, or hormonal treatment in women
- Combined therapies: combination of pharmacological treatment with medical procedures, such as chemical peels, laser therapy, LED light therapy, or mesotherapy
Modern treatment protocols consider not only effectiveness but also safety. Therefore, each treatment plan is tailored to individual patient needs.
Care and diet during acne treatment
Appropriate care is not an addition but an element of therapy. Our dermatologists help select dermocosmetics with anti-inflammatory, moisturizing, and regenerating properties that do not clog pores.
Equally important is a diet supporting acne treatment - rich in zinc, vitamin A, omega-3 fatty acids, and limiting products with a high glycemic index. The patient receives individual recommendations regarding home care during the visit, so that dermatological treatment brings lasting effects and improves the appearance and condition of the skin.
