A testicular hydrocele is a condition involving the accumulation of fluid around the testicle within the scrotum. It may cause enlargement of one side of the scrotum, a feeling of heaviness, or discomfort in everyday functioning. Although in many cases the change develops slowly and does not produce strong pain symptoms, it always requires a urological consultation. At OpenMed in Warsaw (Wola) we offer diagnosis and treatment of testicular hydrocele in adults, with a clear plan of management and a calm discussion of the results.

How does a testicular hydrocele form?

A testicular hydrocele results from a disturbance in the balance between production and absorption of serous fluid in the membranes of the testicle. It may be primary, without an identifiable cause, or secondary - related to trauma, inflammation, infection, or diseases of the testicles and epididymides. For this reason it is crucial not only to confirm the presence of fluid but also to rule out other causes of scrotal enlargement.

Symptoms and when to see a urologist?

The most common symptom is gradual enlargement of the scrotum, usually on one side. It may be accompanied by a feeling of tension, heaviness, or discomfort while walking or sitting. A hydrocele rarely causes acute pain, which is why it is often disregarded for a long time. You should arrange a consultation if you experience:

  • enlargement of the scrotum or a palpable change within it,
  • a feeling of heaviness or discomfort in the testicular area,
  • asymmetry of the scrotum that persists or increases,
  • pain or swelling after trauma or infection.

It is worth remembering that similar symptoms may also occur in other conditions, so accurate diagnosis is of key importance.

What does diagnosis at OpenMed look like?

Diagnosis of a testicular hydrocele begins with a urological consultation. Our doctors take a detailed history, asking about the duration of symptoms, possible injuries, and previous infections. Then a physical examination is performed, which allows initial assessment of the nature of the change.

The basic imaging examination is scrotal ultrasound. It is a safe, non-invasive examination and unequivocally confirms the presence of fluid as well as assessing the condition of the testicle and epididymis. This makes it possible to rule out other causes such as a testicular tumour or inguinal hernia.

Treatment of testicular hydrocele in adults

The method of treatment depends on the size of the hydrocele, the severity of symptoms, and the cause of its occurrence. Small, asymptomatic changes may only be observed. If, however, the hydrocele causes discomfort, enlarges, or interferes with everyday functioning, our specialists may propose surgical treatment.

The most commonly used method is surgical removal of the hydrocele. The procedure consists of removing the accumulated fluid and appropriately securing the membranes of the testicle to prevent recurrence. The procedure is performed in hospital conditions with appropriate preparation and post-procedure monitoring.

At OpenMed we discuss with the patient all the available treatment options, explain the course of the procedure and the possible effects. Thanks to this the decision about treatment is made consciously and without haste.

Follow-up and further management

After completing treatment a urological follow-up is important. Our doctors monitor healing and rule out a possible recurrence of the change. In the case of conservative treatment a plan of observation and guidance on further management is established.

Scrotal ultrasound - the foundation of diagnosis

Scrotal ultrasound is a safe and painless examination that allows a hydrocele to be distinguished from other conditions, including testicular cancer. If you notice scrotal enlargement or a palpable change, do not delay a consultation - early diagnosis is of key importance.

Diagnosis and Treatment of Testicular Hydrocele - FAQ

Most often it does not pose a threat to life, but requires diagnosis to rule out other diseases and select appropriate treatment.

In adults this happens rarely. In many cases observation or surgical treatment is necessary.

The procedure consists of removing the fluid and appropriately securing the membranes of the testicle. It is a standard procedure performed under anaesthesia.

The risk of recurrence is small, but possible. That is why follow-up visits with a urology specialist after the end of treatment are important.

Scrotal ultrasound unequivocally confirms the presence of fluid and assesses the condition of the testicle. Clinical examination alone is not always sufficient to rule out other causes, so imaging diagnostics are essential.

A small, asymptomatic hydrocele usually does not affect fertility. In the case of large changes or coexisting testicular conditions it is worth discussing this with a specialist.

The surgical procedure is performed under anaesthesia. Post-operative discomfort is usually mild and resolves within a few days when the doctor's instructions are followed.
